Students who enrolled after September 28th 2008 who want a paper copy of  Proof of Beginner Driver Education Course Completion need to download the application form and mail it to the MTO at the address on the form, you may also take this form into your local Vehicle Licence Office abnd receive a copy on the spot. Please make sure you get the correct report as they may print an abstract which will not show the drivers education as completed. The bottom of the form should state:- Beginner Driver Education Course Completed (with the completion date).Driving schools will continue to issue certificates to students who enrolled prior to September 28th 2008.

The Driver Licence History Search can only be requested by the licence holder, as it contains personal information. a separate application must be completed and signed by each applicant. The MTO charges $12.00 for the search
